  1. Print and accomplish the Data Privacy Form and Foreign Account Tax Compliance (FATCA) Form.

        i) Data Privacy Form
        ii) FATCA Form
        iii) Client Suitability Assessment (CSA)
        iv) Letter of Consent for REIT Transactions
  2. Fill up the LANDBANK Securities, Inc. (LSI) online trading account opening form by clicking the “Proceed” button below.
    (NOTE: Do not leave any field blank, type NA for not applicable fields.)
  3. Print, fill-up and sign the LSI online trading account opening form and Specimen Signatures..
    4. (NOTE: Printing is required for completing this application.)
  4. Submit the documents along with a photocopy of two (2) valid government-issued IDs to any of the following:

        (1) LANDBANK Securities, Inc.
        16th Floor, The PSE Tower
        One Bonifacio High Street
        5th Avenue corner 28th Street
        BGC, Taguig City; OR;

        (2) Email soft copy of filled-up forms to mystocks@landbank.com
  5. Wait for a call and e-mail confirmation from USI that your online trading account is ready.
